INSURANCE AND POLICIES
Insurance
I am currently a provider with Cigna and Anthem Blue Cross Blue Shield insurance plans. For all other insurance plans I will be considered out-of-network. I will assist you in providing any necessary documentation your plan may require for reimbursement.
Please be sure to contact your health insurance provider prior to coming in for your first appointment. Many insurance companies require pre-authorization for mental health benefits, especially if you are scheduled to see someone who is out-of-network. Failure to contact your insurance company may significantly reduce your benefits. Additionally, please check with your insurance company and be knowledgeable of any co-pays, co-insurance and/or deductible amounts associated with your plan before our first appointment.

Payment
I ask that you pay at the time of your visit unless other arrangements are made. Payment can be made by cash, check, or credit card. I accept Visa, Mastercard, Discover, American Express and Checks should be made payable to Cosgrave Psychology, LLC.

Cancellations
If you need to cancel an appointment, I ask that you contact the office at least 24 hours in advance to be released from your financial responsibility for the visit.
